How to Get a CAP Form 75 (CAP Motor Vehicle Operator ID Card)

 

Overview

A CAP Form 75 (CAP Motor Vehicle Operator ID Card) is required to operate any Civil Air Patrol-owned vehicle.  Not having a CAPF 75 does not restrict you from using a privately-owned vehicle for official CAP business, but is important to have, even if your unit does not have a CAP vehicle.  A CAPF 75 is valid for three years following the date of issue.

Instructions
  • Download SWR Form 75 – Request for CAPF 75. Motor Vehicle Operator Identification Card.
  • Completely fill it out and sign.
  • Attach copies of:
    1. State Driver’s License
    2. CAP ID
    3. 2-year Driving Record (minimum)
    4. Previous CAPF 75 (if applicable)
