How to Get a CAP Form 75 (CAP Motor Vehicle Operator ID Card)
A CAP Form 75 (CAP Motor Vehicle Operator ID Card) is required to operate any Civil Air Patrol-owned vehicle. Not having a CAPF 75 does not restrict you from using a privately-owned vehicle for official CAP business, but is important to have, even if your unit does not have a CAP vehicle. A CAPF 75 is valid for three years following the date of issue.
Sample CAPF 75:
- Download SWR Form 75 – Request for CAPF 75. Motor Vehicle Operator Identification Card.
- Completely fill it out and sign.
- Attach copies of:
- State Driver’s License
- CAP ID
- 2-year Driving Record (minimum)
- Previous CAPF 75 (if applicable)
- Send to:
